Using this tool is a first step in helping you to create your Family Self-Sufficiency Plan.
You and your worker from the Department of Human Services (DHS); Partnership. Accountability. Training.
Hope. (PATH); Michigan Rehabilitation Services (MRS); and/or Michigan State House Development
Authority (MSHDA) will review the results of this tool and make a plan that will
help you reach self-sufficiency.
How to Use the Tool
Navigation
While using this tool,
please do not use the "Forward", "Back", or "Stop" buttons on your browser.
Use the tool's navigation buttons at the bottom of the Web page.

After the Welcome page, each page will have several questions. You must answer every question.
You should select the "Skip" option if a question does not apply to you, or if for some other
reason you do not want to answer it.
WARNING
Do not use the "Backspace" key
unless you are in a text box. Using the "Backspace" key
in any box but a textbox will erase all information you entered on the page. This
could also take you back one Web page.
Tab key
Use the "Tab" key
on your keyboard or use your mouse to move from one field to another to
input or update information. To move backward through the fields, hold
down the "Shift" key
and press the "Tab" key .
Exit
Clicking on "OK" after clicking the "Exit" button
will close you out of the FAST Web application.
If you click the "Cancel" button after clicking on "Exit", you will be returned to
the current FAST Web page to continue.
Textbox
A textbox
is a place for you to type in information. It is the only place you can
safely use the backspace key.
Drop-down Box
A drop-down box
contains a list of possible options. To see the list, click on the down arrow.
Click on the option that best answers the question.
Check Box
A check box
is either checked or unchecked. To check or uncheck the option, click the checkbox
or press the spacebar when it is highlighted.
Continue
Clicking the "Continue" button
will save the information on that page and take you to the next page. This is the
only way to save the information.
Go back
Clicking the "Go Back" button
will take you to the previous page. If you click "Go Back" before you
click "Continue" the information you input will not be saved.
Help
Clicking the "Help" button
will show this help information and is available on each page.
Time Limit
There is a 20 minute time limit for each page. This limit means you have 20 minutes
to complete each page of questions. If you exceed that time limit, you will be
returned to the first page of the FAST and you will have to start over. If you click
the "Go Back" or "Continue" button, the 20 minute timer will start over again.
